HDMF Loan Requirements
BASIC REQUIREMENTS (FOR BORROWER AND CO-BORROWER)
- (4pcs) 1x1 ID pictures with name at the back (photo must be taken 6 months)
- Clear Copy of Two (2) Valid Government Issued ID’s with 3 specimen signatures
- Clear Copy of Two (2) Valid ID’s of Spouse (if married) with 3 specimen signatures
- Clear Copy of PSA Birth Certificate
- Clear Copy of PSA Marriage Contract (if married), Death Certificate (if Widowed), Separation Papers and Liquidation Documents (if Legally Separated)
- Clear Copy of Updated TIN ID
- Clear Copy of Updated TIN Verification Slip
- (Original) 4 Copies Signed Special Power of Attorney (Davao Light Application Format)
- (Original) 4 Copies Signed Authorization (Water District Application Format)
- Member’s Data Form w/ MID# / Pag-IBIG Loyalty Card
- Latest Billing Statement
INCOME DOCUMENTS (FOR BORROWER AND CO-BORROWER) (OF BOTH SPOUSES)
Locally Employed :
- Original and Duly Notarized Certificate of Employment and Compensation (CEC)
- * indicating the gross monthly income and monthly allowances or monthly monetary benefits received by the employee.
- * with company headings, state the current position, date of employment, the status of employment, monthly salary and other allowances and signature of the authorized representative of the company.
- Certified One (1) Full Month Payslip
- * within the last (3) three months prior to date of loan application
- * duly certified by your employer or employer’s representative.
- Latest Income Tax Return (ITR)
- * for the year immediately preceding the date of loan application, with attached BIR Form No. 2316, stamped received by the BIR.
- (Original) 4 Copies Notarized Special Power of Attorney (HDMF Coded Form)
NOTE :
* For government employees who will be paying their loan amortization through salary deduction, the Certified One (1) Month Payslip, within the last three (3) months prior to date of application, must be submitted together with CEC or ITR.
Self Employed :
- Latest Income Tax Return (verified by BIR), Latest Audited Financial Statements and Official Receipt of Tax payment from bank supported with:
- * Latest DTI Registration
- * Latest Mayor's Permit / Business Permit
- Commission Vouchers
- * reflecting the issuer's name and contact details (for the last 12 months)
- Latest Professional Tax Receipt
- Lease Contract and Tax Declaration * if income is derived from rental payments
- Transport Franchise issued by appropriate government agency
- * LGU for Tricycles, LTFRB for other PUVs.
- Certificate of Engagement issued by owner of Business
- Other document that would validate source of income
- * If not working within Davao/Mindanao (cannot personally sign documents)
- (Original) 4 Copies Notarized Special Power of Attorney (HDMF Coded Form)
Overseas Filipino Worker
- Contract of Employment
- * Employment Contract between employee and employer; or
- * POEA Standard Contract (with received stamp)
- * with Name of Employer; Signatures at the end of the contract both parties; Place of Employment; Duration of Contract; Salary; and Position
- Certificate of Employment and Compensation (CEC)
- * CEC written on the Employer/Company’s official letterhead; or
- * CEC signed by employer (for household staff and similarly situated employees) supported by a photocopy of the employer’s ID or passport
- Income Tax Return filed with Host Country/Government
- Payslip indicating income received and period covered
- Valid OWWA Membership Certificate
- Overseas Employment Certificate
- Passport with appropriate visa (Working Visa)
- Residence Card/Permit * permit to stay indicating work as the purpose
- Bank Remittance Record
- Professional License issued by Host Country / Government
- (ADDITIONAL IF SEA BASED)
- * Certificate of Sea Service
- * Valid Seaman's Book Copy
- * History of Employment / Last Crew Contact
- (Original) Consularized / Notarized Special Power of Attorney (HDMF Coded Form) 4 COPIES
- * If locally notarized, attached proof of stay from the principal/s (e.g. DFA sticker of arrival at the back part of the passport/Plane tickets, etc.) 1 COPY
- * If already abroad, a SPA notarized by a Philippine Consular Officer, or SPA notarized by a local notary (of the country where the member is working) but duly authenticated by the Philippine Consulate
NOTE :
* If documents are in foreign language/s, ENGLISH TRANSLATION IS REQUIRED
Online Job Worker :
- Certificate of Employment or contract with letter head indicating company's name and email address
- * in case income is sourced from foreign remittances, pensions, etc.
- Paypal Remittance for 12 months * used for online transfer of fund
- Bank Statements or Passbook for the last 12 months * used for online transfer of fund.
- * If not working within Davao/Mindanao (cannot personally sign documents).
- (Original) 4 Copies Notarized Special Power of Attorney (HDMF Coded Form).
